LPN Tory waited until 8 PM Friday evening to informed me I had only 2 pain meds left knowing Dr. Daly’s office was closed until Monday.
Saturday the meds finished Monday I was told by LPN Tory Dr. Daly was no longer coming to Ivy Stone I should chose Dr. Lisa Rink. I told her I would not be going back and she informed me thinks were going to be difficult to get to a doctor outside I need to choose a Doctor on the compound I declined.
I got Dr. Larry Barr through Medicaid,clerk Natalie made an appointment to see him Tuesday 11/21/17 at 1 PM pick up at 12:15 PM with Sam (ATLAS).
Tuesday 12:15 PM I saw Sam he told me he was not schedule to transport me to Dr. must be another company. I spoke to Nathalie who told me Sam was schedule and she made another appointed for Tuesday 11/28/17 . I visited Dr. Larry Barr he gave me a one week time frame to see Dr. Buck pain management. Tuesday 12/19/17 I was given introductory letter and noticed that it still carries Dr. Lisa Rink’s name and my room as 417 bed #2 which I had left from August 2016. I brought it to attention of nursing staff, LPN Tory told me it was mistake of pharmacy and the front desk. I spoke to front desk she had no control over room change. I was taken to 1500 Midatlantic Dr. Mt Laurel NJ 08054 pain management to see Dr. Buck, was informed he was at a different location and Sam (ATLAS) declined to take me to new location he was not insured to take me from my original location to another I would have to go back to Ivy Stone and have it reschedule. To date I am still waiting to visit pain management.
Five years at Ivy Stone I have used the AC daily to help control my pain. Friday 1/5/2018 about 3:45 AM my roommate Bill William Miller started acting up banging the walls and making noise he shouted at me, “You are a fucking nigger I am going to kill you!” I telephoned the nursing station and left a message on voice mail. LPN Regina returned my call shortly I told her my roommate Bill called me a fucking nigger and threatened to kill me she should send someone to talk to him. She said she would be coming. She came up asked Bill why he threatened to kill me he said because I had the air on, she said she would kill me too because it was cold and turned AC off I had a shower and went down stairs. Not first time I was called a fucking black nigger and threatened to be killed by Mr. Bill Williams Miller, 2nd time within two weeks. First time I called LPN Mike to the room he came with LPN Theresa told them they laughed. Friday I informed Case Manager Natalie she told me she would E-mail Sherron. Friday about 1: 30 PM two aides came to move us I was going to share room with another resident who does not use AC. I called Case Manager Nathalie explained my position and she called Ivy Stone. I was given a new roommate. Sunday Evening about 3:30 PM CEO Sherron, LPN Mike, and Donna came to my room turned off the AC, taped up the wire and taped plastic over the AC I have nothing to help with my pain. I asked CEO Sherron if I was going to get pain meds she said I should talk to my Doctor. I explained to her my dilemma she said she would speak to Clerk Natalie.
I have not received an itemized financial statement for period January 2016 to March 2017 I asked for March 2017 when money was taken from my savings for pharmacy of which I had no knowledge.
Tuesday 1/9/2018 I visited Dr. Larry Barr pain medicine was ordered and he recommended that the AC in my room be turned on re my medical condition, I gave the instruction to LPN Mike who refused to carry out the Doctor’s orders.
Wednesday 1/10/2018 about 8:30 AM CEO Sherron was fired. AC is back working Thursday 1/11/2018.
Has moving date April 1st (all fool’s day) 2018
Went to Social Security to have address change on Wednesday March 14, 2018
I was told IVY STONE was my payee they had placed a doctors’ note on my file I was unable to make financial decisions on my own. I had to get a doctor’s note stating I am capable to make competent financial decisions to get change of address. This was news to me I have been making all financial decisions since I have been at IVY STONE. I never gave IVY STONE permission to make any financial decisions on my behalf.
